Reporting an Issue With An Audit Question

If you believe there’s an issue with a question on your audit, please let us know! Our “Report Issue” feature makes it easy to report a question to Simplifya so we can look into it, make any necessary adjustments, and report back to you.

How it Works

At any point during the audit process, you can report a question that you think has an issue by clicking either the "Report Issue" button while conducting the audit, or the flag icon that appears on each question while the audit is in the Review stage or on the finalized audit report. Upon clicking, a modal will open that will prompt you to explain what you think the problem is.

Upon submitting, we'll receive an email with all the details of the audit, the question, your explanation of why you believe there’s an issue, and your email address.

We will look into your submission and report back to you! If we need to make a change, we will! If not, we'll still be in touch to explain why.
